I use a followme tandem attachment which attaches a child's bike to the rear axle. You can then also fit a child seat. At the moment my 5 year old uses the followme and the three year old is in the child seat. He may outgrow the child seat soon but I think yepp make some seats for older children and...

Another possibility is a followme attachment. I've been using this for about a year now with a younger child in a rear child seat and the 4 year old riding her bike on a followme attachment behind us.

Sks now do a 28in 65mm bluemels which are listed on their website. I bought some from germany (bike-components.de I think) for a surly ogre with 2.3in tyres. Front fitted fairly easily. This question actually prompted me to register as I had a very good experience with them. Only bought a child seat so not a big-ticket item but they were very helpful at dealing with queries on the phone and dispatched it the next day. Thought it was impressive for what is clearly a small outfit.
