Should these be gutted horizontally or at a slight upward angle and if so to what degree? Thanks

For time trial purposes this is covered by CTT rule 14 (competitor's machine) which says that wrists must be no lower than elbows and the height from the ground to the forearm must be no less than 80% of the height of the saddle from the ground.

As for flat or tilted up, look at pictures of TT riders and you will see that some favour flat and some favour tilted up, sometimes at quite an angle.

Hence, for timetrialling, follow the rules which I imagine are similar in triathlon. For other purpses you can do what you want.

Which month/year are you planning on riding in?

Trends change. It was all about curved bars and hands in front of your face then it went all straight and flat and now curved bars seem to be back. I guess its all about what makes your body shape more aerodynamic. Try things and do a roll down test to test variations.
