I can get it to work fine one way (coming down the cassette), but then it doesn't work the other way (going up the cassette). I have to shift 2 up and one down to get it to change. It can't be a mismatch because it's all Tiagra......

Sounds similar (though not quite so bad) as the issues I had (mine was 10 speed). I'd pretty well exhausted everything (i.e. everything else had been swapped and re-setup by highly regarded LBSs) until I swapped the STIs for 105s.

Sounds like hanger bent, or gear cable fixed wrong side of derailleur...

Two good bike shops checked the derailleur with their special machine. Problems had been bad pretty well since new so if cable fixed the wrong side then Genesis started it by assembling it like that and then a couple of excellent LBSs (and a few not so excellent ones) failed to spot it whilst trying to resolve the issue.
