Both 1" and brand new in the boxes - I no longer have any bikes with threaded headsets and I unearthed these when having a tidy-up/throw-out of stuff in my workshop (realiy is that I'm just packing the stuff a bit tighter on the shelves!)

I would imagine if you want to 'bite someone's hand off' when they offer a cracking deal you do so via pm and/or by a definite 'yes, yes, yes, I will buy it now'.
Responding to an ad with a question can hardly be construed as an offer to buy.
If I wish to sell an item I will generally give it to the first person who offers the asking price, not the first person who responds to the ad.
I bought a ''showers pass" refuge jacket on here a while back for sub £15. I wasn't sure if it was the right size but asking questions regarding; pit to pit / ape index / cuff circumference etc would mean I would definitely have lost the deal. If you see a good deal then just offer to buy it, if it ain't right then just sell it on again.
I would imagine others may have differing opinions, but I do not think there is a set protocol for selling on here.

Your first 3 sentences just sum it up very well (and is the gist of my PM referred to) - there were 3 people wanting to buy the item prior to the question post - the first person has paid and it's winging its way there (a further 2 people responded later). First come, first served - that's the fairest way.
If I see something on a for sale forum I always send a PM - makes sense.

I always do both and if there's multiple items in the advert, say in the thread which the PM is about. I think it saves others wasting their time. When I've advertised in several places I also say so. There's no obligations of course, on either side, offers to sell and offers to buy are just that, the deals not done till the money changes hands.

I usually do send a pm but my post was a request for information too. There are two distinct headsets. The brushed silver is aluminium and has roller bearings. My request for clarification was better put to the forum so that if it wasn’t what I wanted any other forum member interested would have clarification too.
