If you want a custom made bb / axle try Mandini, (google him and his you tube channel) a cycle engineer based in Poole, Dorset. Very high quality and will, if you ask nicely, try and replicate your defective one.

I had a similar Mavic item many moons ago and it worked for many years. The Mavic one cost twice that 20 years ago! needed a 45 degree taper cutting on the BB shell, and it looks like that one does as well.
