Bell doesn't cut it, decent alternative?
Thinking a courtesy tone and a moped loud horn to alert the ear bud wearing and brain dead that are oblivious-prefer not to run them over as it could damage the bike.

Get one of the big dutch ones .. works for me ..

Mrs Syd and I both have Crane E-NE Bells on our commuting bikes.
I find a single ping from 20 , or so, metres away attracts the attention of most pedestrians.

For people with headphones and behind car windows, I use the horn that charges with USB: https://www.aliexpress.com/item/1005001646440191.html. There is an alarm version there, but apparently that alarm is marginal. There are other vendors of that horn, including on Amazon.

I have a very loud single ping bell. People hear it but I think that no one seems to link any kind of bell with bicycles anymore.

I have Lion Bellworks on both my bikes. Nice clear loud ping.
Riding along a lane once, I pinged politely approaching two ladies walking in the same direction as I was riding. Nothing.
I pinged a little bit more urgently. Nothing.
I pinged and pinged and pinged and pinged ................. one lady jumped out of her skin and said, "I wondered what that ringing noise was!"
True story.

I know what you mean - I ping my cheap pingy bell with gusto but sometimes folk simply don't hear it - specially when they're chatting. So I was well impressed when I saw my neighbours new aquisition - a fancy chrome bulb horn, which I can confirm is well louder than my silly little pingy bell. Halfords apparently...

This is why I'm thinking courtesy bell polite for 90% followed by HGV horn loud for the 10% terminally stupid.
Looked at lion bellworks and shop closed.
There are spur cycle knock off bells on eBay for £6
So far thinking a nice bell and air zound combo

If your bell doesn't work, waiting always does. But going out without considering them brain dead probably works best of all. If you don't want to mix with pedestrians, stick to the road.

Just about all my issues with pedestrians are on the road!
Crossing in town, and walking along unpavemented roads.
This is the worst bit in Tavistock.

I have very loud pawls in my Hope Rd40 wheels
Just freewheel about 20ft from someone and they hear them
They're not as loud or industrial sounding as the old Hope Pro2 mtb hubs but they're definitely a distinct noise

My experience, and I assume Mick F is too, is on the road. Quiet back lanes, people walking spread out across the road..... oblivious of cyclists.
I think folk are now unaccustomed to cycle bells of any kind.
