rim failure

I have seen many references to rim failure.
I recently had a front rim fail while the bike was parked in the garage
(I feel very lucky I wasn't riding it). see

It had a fairly knobbly mountain bike tyre at about 65 psi.
(I was getting the bike to change to a new tyre when I found the rim failure.)

Failure from the join outwards.
Rim is a Rigida Turbo 90.
Thickness of rim along failure: 1 mm.
Thickness a rim before it crochets out: 1.3 mm.

About 10 years old, used for about 5 of those for dirty commuting
(part offroad, and not cleaning rim/brake regularly).
So quite a bit of wear is reasonable.
