I’ve got an extremely arthritic ankle that is a consequence of a climbing accident 30 years ago.
Anyway I use flat MTB pedals which works totally fine for me (e.g. I can cycle all day every day).
I don’t get significant pain during or after cycling.
Haven’t tried any type of SPD to compare it to though.
Whereas my walking is pretty limited & I suffer the next day if I do a 10 mile walk for example.
I think it helps to be able to slightly vary my foot position during a ride
