It depends on your rear gear ends and the drop they give between the hub centre and the centre of the gear mounting point. I used a Shimano Deore DX long cage with a 14/38 5 speed freewheel for several years without problems. This was on a Holdsworth frame with Simplex rear drop-outs. I suspect from memory that these had a deeper drop than my present Campag drop-outs. I now use a Huret Duopar on a 5 speed 14/38 freewheel using a frame with Campag drop-outs. This works fine and even indexes faultlessly with Suntour 5/6 speed levers - in my case mounted on Kelly Take-offs. I also use a Suntour XC Pro long cage rear mech on Campag drop-outs with a 7 speed 14/36 cassette, but I don't think it would stretch to a 38 tooth sprocket. Modern Deore rear mechs will also take 36 tooth sprockets and I suspect would stretch to a 38 tooth, but I have not tried this.

Any of the Suntour mechs labelled as AG (Alpine Gear) were designed to cope with the 38t freewheel cog.

Suntour AG and Huret Duopar I can vouch for.
