I know what you mean. I removed a pair of M540 pedals that had been on a daily commuter for years. It was not easy. I have 3/8" drive allen bits now, so I can use a long ratchet handle.

Yes I used a long ratchet handle to remove Allen bolts to remove worn out SPD cleats. Means you can use more leverage on a small area. Worked where using the combi driver tool failed.

update; it seems that Shimano now sometimes supply spacers with their SPD cleats;see
