Clue; it came out of a wheel which hadn't been pranged....

Imbalance in tightness of spokes?

Too much spoke tension on one side of the wheel? i.e 12 o'clock to 3 o'clock.

nope....

another clue; it may look as if the hub barrel is slightly fatter in the centre; appearances may not be deceptive in this case...

Axis of bearing surfaces and axis of thread not co-incident.

Cone precessing and crushing the hub....in my experience, it only ever happens when the hub is used as shown, with the drive on the left.... :wink:

how do you think this happened?
