Too much spoke tension on one side of the wheel? i.e 12 o'clock to 3 o'clock.

nope....
another clue; it may look as if the hub barrel is slightly fatter in the centre; appearances may not be deceptive in this case...

Cone precessing and crushing the hub....in my experience, it only ever happens when the hub is used as shown, with the drive on the left....
